New patients are always welcome at Tampa Dental. Your initial appointment will consist of x-rays and a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, most often, a medical history or treatment plan will require additional appointments. Following your initial appointment you will be given an appointment with one of our hygienists for a teeth cleaning and you will be scheduled for your follow-up appointment if further treatment is needed.

Please assist us by providing the following information at the time of your consultation:

  • X-rays (if applicable)
  • New Patient Forms
  • Dental Insurance Card and Photo I.D.

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern. (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If you have x-rays from a previous dentist, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
